Dans des recherches men\u00e9es chez des rats, les chercheurs ont d\u00e9couvert que la double inhibition du SGLT1 et du SGLT2 r\u00e9duisait plus efficacement l&rsquo;hypertension sensible au sel et les l\u00e9sions r\u00e9nales que l&rsquo;inhibition du SGLT2 seule. Les r\u00e9sultats seront pr\u00e9sent\u00e9s lors de l\u2019ASN Kidney Week 2025 du 5 au 9 novembre.<\/p>\n<p>L\u2019hypertension sensible au sel \u2013 une pression art\u00e9rielle \u00e9lev\u00e9e due \u00e0 une consommation excessive de sel \u2013 touche pr\u00e8s de la moiti\u00e9 des personnes souffrant d\u2019hypertension art\u00e9rielle et contribue consid\u00e9rablement aux maladies r\u00e9nales, aux complications cardiovasculaires et \u00e0 la progression vers l\u2019insuffisance r\u00e9nale.<\/p>\n<p>Lorsque les chercheurs ont compar\u00e9 l\u2019inhibition s\u00e9lective du SGLT2 (par le biais d\u2019un traitement \u00e0 la dapagliflozine) avec la double inhibition du SGLT1\/2 (par le biais d\u2019un traitement \u00e0 la sotagliflozine) dans un mod\u00e8le de rat bien \u00e9tabli d\u2019hypertension induite par le sel et de maladie r\u00e9nale chronique, ils ont constat\u00e9 que les deux m\u00e9dicaments avaient des effets profonds sur le ralentissement de la progression de l\u2019hypertension induite par le sel.<\/p>\n<p>Par rapport \u00e0 l&rsquo;inhibition s\u00e9lective du SGLT2, la double inhibition du SGLT1\/2 a produit une r\u00e9duction plus importante de la pression art\u00e9rielle moyenne et att\u00e9nu\u00e9 plus efficacement les l\u00e9sions r\u00e9nales, bien qu&rsquo;elle n&rsquo;ait eu aucun impact sur la pression art\u00e9rielle dans des conditions normales de sel. La sotagliflozine a \u00e9galement r\u00e9duit le poids corporel, am\u00e9lior\u00e9 l&rsquo;excr\u00e9tion urinaire de sodium et de chlorure et presque doubl\u00e9 l&rsquo;excr\u00e9tion fractionn\u00e9e de glucose par rapport \u00e0 la dapagliflozine.<\/p>\n<p>Aucun des deux traitements n\u2019a modifi\u00e9 la fonction r\u00e9nale. L&rsquo;inhibition du SGLT2 a modul\u00e9 plusieurs voies m\u00e9taboliques d&rsquo;une mani\u00e8re sp\u00e9cifique \u00e0 une r\u00e9gion du rein, avec des effets prononc\u00e9s sur le m\u00e9tabolisme lipidique et la signalisation inflammatoire.<\/p>\n<p>\u00ab\u00a0Notre \u00e9tude fournit des preuves pr\u00e9cliniques soutenant l&rsquo;utilisation \u00e9largie des doubles inhibiteurs du SGLT1\/2 au-del\u00e0 de l&rsquo;insuffisance cardiaque et du diab\u00e8te, \u00e9tendant ainsi leur potentiel \u00e0 la gestion de l&rsquo;hypertension, en particulier chez les patients sensibles au sel\u00a0\u00bb, a d\u00e9clar\u00e9 l&rsquo;auteur correspondant Olha Kravtsova, Ph.D., de l&rsquo;Universit\u00e9 de Floride du Sud.<\/p>\n<p>\u00ab\u00a0Ces r\u00e9sultats constituent \u00e9galement une base pour des recherches plus approfondies sur le m\u00e9tabolisme r\u00e9nal r\u00e9gional.
